LIABILITY WAIVER 

BACK TO SPORTS + BACK TO SPORTS MOBILE 

The undersigned expressly agrees that all use of any Back To Sports facilities including but not limited to the mobile unit/bus, exercise equipment, exercise machines, and treatment areas is undertaken at his/her sole risk. The undersigned further agrees that Back To Sports is not liable for any injuries, accidents, illness, death or damages to the undersigned or the undersigned guests), as well as any property of Back To Sports is not subject to any claim, demand, injury or damages whatsoever, including without any limitation those damages resulting from passive negligence on the party's owners, officers, agents or employees. The undersigned for himself/herself and on behalf of his/her executors, administrators, heirs, assigns and successors indemnifies and holds harmless Back To Sports from all such claims, demands, injuries, damages, actions or causes of actions. The undersigned agrees that Back To Sports is not responsible for or liable for articles damaged, lost or stolen in or about any Back To Sports facility or mobile unit.
